Dental benefits will be administered by Ameritas .

Choosing an in-network Ameritas dentist will stretch your annual maximum further and reduce the chances of balance bills that are common with out-of-network dental providers.

Nominate your Provider!
If you would like your dentist to consider joining our network, please provide us with the appropriate information below. We will reach out to the office and invite them to join our PPO network. Please note: Inviting a dentist to join our network does not guarantee they will become a participating provider.
